You can access disability support with our person centred support services. We work in close partnership with individuals with disability, their families and support networks to establish a positive collaborative relationship. We provide consistent and coordinated support to identify, link with and organise the supports required to enhance the quality of life for you, your family or carer.

Our case managers will help you develop a plan to achieve your goals and aspirations, and work with you to help make these dreams come true.

Our assistance includes:

  • transitioning from hospital to home 
  • connecting with and accessing local services and networks
  • finding the right service e.g. in-home care, counselling and leisure
  • finding a job
  • finding a place to live
  • living independently 
  • identifying eligible funding support
  • assisting with funding applications 
  • working with family members or support networks to resolve issues
  • advocacy

This is a free service to all eligible consumers with a disability or the elderly who meet our funding criteria, with no limits on age. Our service can also be provided on a fee for service basis for anyone who is not eligible for the free service. If you’re ineligible, our case managers can provide you with information on alternative services.

You don’t need a formal referral, so you can contact us directly or be referred to us by your carer, health professional or other service provider.

Please note we try to grant priority access to people who have the least ability to access services or recreation opportunities independently due to:

  • having severe or multiple disabilities
  • geographic or social isolation
  • lack of personal support networks
  • people who are not currently employed, in training or a day service
  • whose support arrangements are in jeopardy
