Case Management Service


Independence Australia provides case management service for people with a physical disability. We specialise in working with people who have a disability and have particular expertise in the area of Spinal Cord Injury and Post Polio. Our case managers work alongside clients and their families to address personal goals which are client driven. Case managers have tertiary qualifications in social work or related fields, and our work is based on a person-centred framework of practice. The main purpose of case management is to enable clients to live as independently as possible in the community.
Case managers comply with the Case Management Society of Australia’s (CMSA) Standards of Practice and the Australian Association of Social Workers (AASW) Code of Ethics.
Accessing Case Management Services:
  • Any person with a physical disability or older person can apply for service from Case Management
  • Referrals can be made by individuals, family members, carers, health professionals and other disability service providers
  • There is no age limit
  • The service is free to clients
  • All clients accepted for case management services will be placed on the waiting list and advised of approximate commencement date of services.
  • If a client is ineligible for services Independence Australia case managers will provide information regarding alternative services
Case Managers may provide support with:
  • Following a spinal cord injury or hospital admission, we provide case management support with transitioning from hospital to home
  • Accessing services within the community
  • Linking individuals with specialised training and employment services
  • Providing information on, and accessing, recreation and leisure services and public transport
  • Assisting with funding applications for recreation/attendant care hours
  • Advocacy
  • Any other personal goals set by clients which will enhance their quality of life and community participation.
  • Liaising and negotiating support and services with clients who receive compensation through TAC or Work Cover Insurance agencies.
How we provide Case Management:
  • Coordinating community, social, health, recreation services to ensure that clients and their families are receiving
    appropriate supports and services to meet their individual needs.
  • Work with family members, carers, health professionals and other disability service providers.
  • Support the researching of information and decision-making regarding life choices and goals.
  • Make referrals to other disability/community services to meet the needs of the client.
  • We encourage and support individuals and their families to self advocate.
  • If required we utilise interpreting services.
